Peace of Mind for Life – Level 1

Peace of Mind for Life is designed to engage you in conversations about end-of-life care choices. The intention is for every person, regardless of age or health status, to make well-informed choices and to complete their Advance Medical Directives. This will assure that your chosen Health Care Proxy fully understands your wishes as directed in your Living Will.

Level 1: What, How, Who, Why
You will leave with a better understanding of what your choices are and will have the necessary forms to complete the process.

(Attendance at Level 1 is a prerequisite for Level 2.)
